How to read this table
The columns are modalities, not quality scores. A filled circle means the provider includes that modality in at least one published tier, which is not the same as including it at the entry price. Read the circle next to the tier breakdown in the provider notes below, never next to the entry price on its own. Two providers in this table gate specific modalities behind higher tiers: YEARS puts whole-body MRI and its liquid biopsy in the €7,600 Evolve programme rather than the €1,900 Core one, and Biograph reserves its multi-cancer blood test for the $15,000 Black membership. A circle also says nothing about how well a modality is delivered, or whether it should be there at all. Two of these columns cover things we grade poorly on evidence, so read them alongside the grades rather than as a shopping list.
“Entry price” is the lowest published price for a single adult, excluding insurance reimbursement, and for membership products it is the annual fee. We do not convert currencies in the table, because the exchange rate on the day you read this is not the exchange rate we used. The internal sort key is an approximation and is not published as a price.
The last column is the one we would look at first. “Published” means the provider states the price on its own site. “Reported” means it does not, and the figure comes from a named third party. A provider that will not publish a price is telling you something about how the consultation is going to go.
Provider notes
Three published tiers on one Berlin site, each delivered as a single supervised day. The tiers gate modalities rather than only depth, and the structure is worth reading closely: Core at €1,900 is a functional and laboratory workup with no imaging, no liquid biopsy and no genetics, built around spiroergometry, echocardiography, vascular ultrasound, cognition and lung function. Whole-body MRI and the multi-cancer liquid biopsy both arrive at Evolve, €7,600. Genome sequencing, epigenetic clocks and microbiome analysis arrive at Ultimate, €16,900. Two observations follow. Core is the most evidence-aligned entry tier in the Index, because it leads with the modality we grade highest and omits the one we grade C. And Evolve is the cheapest programme anywhere in the Index that includes whole-body MRI and an MCED test together, against $15,000 for the equivalent US tier at Biograph.
Core, €1,900. Spiroergometry, ECG, echocardiogram, vascular ultrasound, cognition test, lung function, body composition scan, physician review. 87 biomarkers. No imaging, no liquid biopsy, no genetics.
Evolve, €7,600. Everything in Core, plus 3T whole-body MRI and a multi-cancer liquid biopsy (MCED). 120+ biomarkers. One annual check-in. No full genome sequencing.
Ultimate, €16,900. Everything in Evolve, plus genome sequencing, epigenetic clocks (DunedinPACE and GrimAge), microbiome analysis. 230+ biomarkers. Three annual check-ins.
A proprietary optical scanner for skin, a cardiovascular sensor array and a blood panel, delivered in about 60 minutes for £299 in the UK. No imaging by design. Over 100,000 examinations completed across Sweden and the UK, roughly $1.03bn raised, and around three-quarters of members prepay the following year. The bet is annual attendance rather than diagnostic reach, and it is the only model in the category whose core measurements all have decades of outcome trials behind them.
Restructured into three memberships: Core at $1,199 with a focused scan and lab panel, Comprehensive at $2,499 with a whole-body scan and detailed labs, and Executive at close to $5,000 adding brain health and body composition. Over 170,000 scans and 29 clinics make it the largest screening-MRI operation in the category, and its first European site opened in London in 2025. The focused scan excludes spine and limbs, which is worth checking against the tier you are quoted.
A blood-panel subscription that acquired its imaging arm: Function bought Ezra in 2025 and now sells members an MRI at $899, or $999 publicly, across more than 170 partner imaging sites. Scan time runs 22 to 47 minutes depending on protocol, and the entry protocol is not whole-body. Roughly $350m raised by Function, with about $43m into Ezra before the acquisition.
Core membership at $7,500 and Black at $15,000, the difference being a multi-cancer blood test and additional check-ins through the year. Bundles MRI, DEXA, VO₂ max and genetic testing, with CT coronary angiography available. Around $54m raised. The closest US structural analogue to the European outpatient programmes, at four times the entry price of the Berlin equivalent.
Core, $7,500. MRI, DEXA, VO₂ max, genetic testing. CT coronary angiography available. No multi-cancer blood test.
Black, $15,000. Everything in Core, plus a multi-cancer early detection blood test and additional check-ins through the year.
Sold as CORE, APEX and APEX Family annual memberships, with APEX reported at roughly $19,500 to $21,500 a year. The most extensive diagnostic bundle in the Index and the most expensive. Diagnosis and in-house therapeutics sit within the same business, which places more of the interpretive judgement inside the organisation providing the treatment. Members should read the referral pathway carefully; the model works well for people who want one team owning everything.
MRI plus 120-plus biomarkers, a 163-gene panel and wearable data, closing with a 45-minute physician review. Around $80m raised. No list price is published, so a prospective patient cannot compare it against anything without booking a consultation first.
A blood-panel membership with a consumer app layer and no imaging, at the same price as Function’s subscription tier. Competes on interface and longitudinal tracking rather than diagnostic reach.
Whole-body MRI through a partner radiology network with a radiologist report and no clinician wrapped around it. The floor case for the category: this is roughly what the scan costs when nothing else is bundled with it, which makes it a useful yardstick for pricing everyone else’s bundle.
Clinician-led programme combining imaging, extensive bloodwork and functional testing across several appointments rather than one day. The closest structural match to the European outpatient model outside Europe.
One of the older German private check-up centres, working through a specialist physician model. Reported packages run roughly €1,400 to €2,000, with a Basis Check covering internal examination, laboratory diagnostics, 4D ultrasound and rest and stress ECG, and Basis Plus adding MRI of two organs where clinically indicated. Organ-targeted rather than whole-body imaging, which is a defensible position on the evidence and a narrower one commercially.
Three resorts on the same medical method: Lans in Austria reported from €5,390, Sylt from €6,480 and Tegernsee, the flagship, from €8,435, each for seven nights including the Classic medical programme and the least expensive room. Classic Plus extends the diagnostic component to a reported €6,440 at Lans and €7,546 at Sylt, and Sylt sells a four-night Longevity Check from €4,844. The method centres on the Lanserhof Energy Cure, modified fasting and gut treatment, with diagnostics supporting a stay rather than being the product. Lanserhof publishes prices only inside downloadable PDF brochures, so figures here come from a named secondary source and reported entry points vary with room category and season. LANS Medicum in Hamburg and Lanserhof at The Arts Club in London run outpatient check-ups and concierge medicine on the same brand without the residential element.
The most expensive programme in the Index. Reported seven-night pricing runs from about CHF 20,950 for Detox to CHF 31,800 for Revitalisation and CHF 50,250 for Revitalisation Premium, including accommodation, medical consultations and the proprietary CLP Extract. Operating since 1931, which makes it the longest-running business in the category by a wide margin. The signature treatment is the part with the least published evidence, and the medical workup around it is conventional.
Longevity programme reported at €12,270 for seven nights, €16,700 for ten, €21,840 for fourteen and €26,810 for twenty-one. The tapering per-night rate over longer stays is unusual in the category and suggests the programme is designed around a multi-week intervention rather than a diagnostic episode.
Entry pricing reported from around CHF 8,000 for a seven-night stay on the Chenot Method, a structured detoxification and energetic treatment protocol. Positioned between the Lanserhof resorts and Clinique La Prairie on price.
The most accessible entry point among the residential clinics, reported from around €3,500, with an Advanced Longevity health programme at about €1,200 per guest per night before accommodation. Macrobiotic nutrition is the organising idea rather than diagnostics.
